12S BIRTH CERTIFICATE

12S - Nationals born neither in US nor in Algeria

a/ If the applicant`s birth is transcribed at the Embassy / Consulate of Algeria in the country of the applicant`s birth.
-Send to Algerian Embassy in Washington - D.C a copy of the applicant ID or the one of the tutor.
-Send to Algerian Embassy in Washington - D.C a copy of the Family book in case of its availability.
-Submit to Algerian Embassy in Washington - D.C the written application asking for 12-S.
b/ If the applicant`s birth is not transcribed at the Embassy / Consulate of Algeria in the country of the applicant`s birth
- Send to Algerian Embassy in Washington - D.C the original birth certificate
- Send to Algerian Embassy in Washington - D.C a Descriptive form downloadable at our website in CIVIL STATUS to be filled in Arabic and French )
- Send to Algerian Embassy in Washington - D.C a copy of the applicant ID or the one of his tutor.

In both cases, The Algerian Embassy in Washington - D.C will forward the application to the suitable Algerian Embassy/consulate where normally the applicant`s 12-s owes to be issued (i.e. Embassy / Consulate of Algeria in the country of the applicant`s birth.) via The Ministry of Foreign Affairs in Algiers.
